    DOH-2832 (Dept. of Labor Copy)

This is what needs to go to the NYSDOL to get your license!

You will need to sign this where indicated (if you didn’t already do it in your In-Person class) and submit this to the NYSDOL with your application and the appropriate fee made payable to the Commissioner of Labor.

    1. To apply for the Company License, go to the Lead Renovation/Abatement Firm Certification Application

    2. Enter your Company's Name, Address and Phone # and click the Search Button (the USEPA’s application process makes you search for your firm before it will open an application).

    4. Follow the on-screen instructions to submit your application.

Your Certification must be renewed every 5 years by taking a 4-Hour USEPA Lead Certified Renovator Refresher.

For Lead Inspectors, Risk Assessors, Supervisor and Abatement Workers:

The Certificate of Completion we sent you is your Interim Certificate which allows you to work immediately - but is only good for 6 months!

We strongly recommend you apply for your Full License with the USEPA as soon as possible.

You must have the following documentation available to apply:

To apply for your Full License:

    1. Go to the Individual Certification Information and Search.

    2. For first timers, select "No" next to "Do you have a current or previous certification?". (For Renewals - select "Yes").

Note: Lead Inspectors, Risk Assessors and Supervisors are required to sit and successfully pass an EPA exam.

Your Certification must be renewed every 3 years by taking the appropriate 8-Hour USEPA Lead Refresher.

    IMPORTANT:

All Lead Inspectors, Risk Assessors, Supervisors and Abatement Workers must also work for a certified Lead Activities Firm!
